Requirements
Interested SGS applicants shall submit the following requirements to the Channel Sales Supervisor:
- Photocopy of Valid Proof of Identification (POI)
- Photocopy of Valid Proof of Address (POA)
- Two (2) ID picture *
- Original NBI and unexpired clearance with thumb mark and receipt (official receipt) for NCR or Original Police Clearance and Original Barangay Clearance for Provincial areas not covered by NBI Satellite Offices. **
- Accomplished and signed Smart Gold Specialist Information Sheet and Terms & Condition.
Notes:
*Passport size (2x1.5), blue background, with collar, recent (at least one month), colored.
**NBI Renewal Card shall NOT be considered as a substitute for the Original NBI Clearance.
