Travel Accident Insurance
Provides lump sum benefit for death or permanent disablement due to accident suffered while traveling outside the Philippines during the period of cover.

Delayed Flight Protection
Reimburses expensed for meals, refreshments. Hotel expenses and travel costs if flight is delayed for more than 6 hours due to severe weather conditions, strike or airline personnel or equipment failure of aircraft.

Cancelled Flight Protection
Pays for loss of travel and/or accommodation expenses paid in advance up to the maximum limit specified in the policy and not refundable in case trip had to be cancelled due to any of the following events occurring within 30 days before the scheduled departure date:
• Any cancellation by an air common carrier caused by inclement weather.
• Any cancellation due to a strike or other job action by employees of any air common carrier scheduled to be used by the insured person during his/her trip
• Any cancellation caused by equipment failure of an air common carrier

Travel Misconnection
In the event that the insured Person’s confirmed onward travel connection whilst overseas is missed at the transfer point due to the late arrival of connecting scheduled conveyance and no onward transportation is available to the insured Person within 12 consecutive hours on his/her arrival, Philam Insurance Company, Inc (Philam) will pay for actual additional expenses necessary and reasonably incurred for accommodation, restaurant, meals and refreshments.

Delayed Luggage Protection
Reimburses purchase of necessary clothing and toiletries (up to a maximum stated in the schedule of benefits) if checked-in baggage is delayed for more than 6 hours.

Lost Luggage Protection
Philam shall pay the insured Person up to the limit stated in the schedule of benefits for loss to the insured Person’s checked-in luggage. Loss must occur while the checked in luggage is within the control of the common carrier’s.

Lost Travel Documents Protection
Reimburses costs of additional hotel, travel, and communications expenses necessary in obtaining replacement of lost passport or visa as a result of robbery, burglary, or theft while overseas.

Exclusions:
This plan shall not cover any loss resulting from childbirth, miscarriage or complications therefrom, routine physical examination, cosmetic or plastic surgery except as a result of injury, any mental or nervous disorders, congenital anomalies, suicide or self-inflicted injuries, war, military service and pre-existing conditions. This policy will not cover any loss, injury, damage or legal liability arising directly or indirectly from travel in, to or through Afghanistan, Cuba, Democratic Republic of Congo, Iran, Iraq, Liberia, Sudan, or Syria.

Claims Procedure. In case of a claim, here are the steps to be followed:

1. Kindly contact Philam Insurance Company, Inc. (Philam) Claims Department at 878-5463. They will provide you a claim form.

2. Fill out the claim form and attach necessary documents. For claims for flight delay, loss/damage to baggage and baggage delay, you must file with Philam within 120 days from the date of your return from your trip.
 
Basic Claim Requirements
a. Smart billing statement with international roaming charges within the first 5 days of your trip. This requirement is exempted in case of death while en-route to destination outside the Philippines.
b. Travel Claim Form
c. Photocopy of passport
d. Photocopy of airline ticket / boarding pass

Accidental or Death Disablement
a. Police report
b. Birth certificate
c. Death Certificate
d. Post Mortem Report / Autopsy Report
e. Marriage Contract
f. Physician’s report and medical records (relative to the accident)

Flight Delay
a. Certification from the airline / carrier stating scheduled departure time, actual departure and the reasons for delay of flight.
b. Official receipts of expenses incurred due to flight delay.

Cancelled Flight
a. Common Carrier's Cancellation Notice
b. Certification/Affidavit stating the reason for the trip cancellation
c. Documents to support the reason for the trip cancellation
d. Official receipts of expenses incurred due to trip delay/cancellation

Baggage Delay
a. Property irregularity report
b. Receipts of necessary emergency clothing and toiletries due to baggage delay.
c. Proof of delivery / receipt of baggage from the courier / airline.

Lost / Damage to Baggage
a. Claim and complaint report against airline / carrier or other authority or individual responsible for the loss / damage
b. Receipts for items being claimed
c. Property irregularity report
d. Photos of damaged luggage / items

3. Kindly submit claim form and necessary documents directly to Philam.

4. Philam process claim – 5 working days upon receipt of complete documents and certification for simple claims / 5 working days upon receipt of final report from claims adjuster for cases requiring further investigation.
a. If claim is approved, Philam will inform you and releas claims proceeds directly.
b. If claim is denied, Philam will inform you of denial and release denial letter.
c. If there are additional documents, Philam will inform you of additional documents.
